(nee Moyer) Kozak; devoted father of Leonard Kozak and Bridget (Michael) McLellan; cherished grandfather of Michael, Zachary and Krista; loving son of the late Stanley and Anna (nee Palka) Kozak; dear brother of Stanley (Maryann) Kozak, Ann (late Willard F.) Still, Frances (late F. W. John) Radford and the late Henry Kozak. No Prior Visitation. Memorial Mass will be celebrated at a later date.
