Beloved husband of Carol (nee Kaczmarek) Derylak; devoted father of Michael Derylak, Jennifer (Aaron) Clark, Michael (Annette) Kozak and Lisa (Joseph) Schaffer; cherished grandfather of Anai, Cora, Gabe, Jake, John, Joe, Rhythm and the late Orion; loving son of the late John and Bertha (nee Szaikowski) Derylak; dear brother of the late Tina (Paul) ODonnell; also survived by many friends and his beloved dog, Jasmine.
